Below is a link to a survey that asks your opinion about using telehealth services. Telehealth includes providing services using a telephone, smart phone or computer. Telehealth can also be provided using video conferencing which are meetings conducted using video and/or audio to communicate.
This survey is anonymous so we will not know who is filling it out. We will use the information collected from these surveys to determine if telehealth services should continue to be provided after COVID-19 is no longer an issue.
