Community Electronic Health Record (CEHR) Portal

A Community Electric Health Record (CEHR) account allows you to see information like past and upcoming appointments, lab test results, medications, resources, and more.

Setting up your CEHR portal account

Before you can set up an account you will need your Case Number and PIN which is a separate document to these instructions. You will receive it from a WCCMH staff member. 

Once you have your Case Number and PIN, you can set up your CEHR portal account here:

You'll create an account with your name, email, birth date, case number, and pin. You'll be promoted to create a username, password
