School Health
The Michigan Public Health Code requires schools, preschools and childcare centers to report student immunization levels and communicable illnesses to their local public health department. Requirements apply to both public and private schools and childcare settings.

Communicable Disease Reporting
Schools and childcare settings are required to report cases and outbreaks of communicable diseases to the Health Department. Please use these forms for reporting:
- Use this form for outbreak reporting: bit.ly/outbreakreport. This form should be used anytime camps/schools are aware of an outbreak that exceeds 20% of a particular group (e.g., a classroom).
- Use this form to report communicable disease cases. This form should be used anytime you have a case of any communicable disease where reporting is required with even a single case (e.g., measles, pertussis). These require immediate reporting. For a list of reportable diseases, see the table on pp. 10-14 of the Managing Communicable Diseases in Schools guidance.
Immunization Reporting
Immunization reporting utilizes the Michigan Care Improvement Registry/Schools Immunization Reporting System (MCIR/SIRS).
